Example Annual Assessment Template

My first task working with the Indiana University Online’s assessment team was to create a set of templates that reflected the standardization and integrity that the office wanted. Working with stakeholders, I created a template that reflected the interests of the 54 different program committees. The basis for the annual report was created by a previous team and was focused primarily on grade data. For assessment work, this is an inaccurate and ultimately misleading practice. Working with my supervisor, the program committees, and the advising group, I revised the template to reflect a more complex data set. In the next iteration of the report, I incorporated branding with input from the Marketing and Compliance departments. In the most recent version (below), the assessment team wanted to streamline the process and combine two templates into one document. Programs that were participating in the assessment process but had not collected data completed a different document: the annual review. I merged the annual report and the annual review seamlessly to include all information with no redundancy.
